Our Integrative Medical Center is located on Lake Merritt in downtown Oakland, California. We are inside the iconic Kaiser Center and there is ample parking. Location is at the intersection of Harrison where it becomes Lakeside, between 20th and 21st Streets. See link below for details. Please complete the intake form (link below) and bring it with you. If you have recent lab work, please bring COPIES of those results. Please note that HIPPA requires that copies of tests and physicians notes may only be released with your written request. Bring with you supplements and prescription medications you are currently taking. We do not bill insurance directly but provide an insurance "superbill" upon request that you may send to your health insurance provider for their contribution. All fees are due and payable at the time of your visit, and we require a credit card to hold your appointment. Please note our 48-hour cancellation policy. We will attempt to fill cancelled appointments -- if successful, there will be no charge. If you need to change, provide as much notice as possible to help us to accommodate you. If less than 48-hour notice is given, and we are unable to fill the appointment, you are responsible for all fees. If you have questions, please call or email and we will assist you.
